After the Great Depression, Crownsville Hospital became severely overcrowded, underfunded, and chronically understaffed, producing horrific patient conditions. In 1949, the Baltimore Sun and the Baltimore Afro-American published detailed incidents of patient neglect and abuse, and unrest followed as patients at several points in the early 1950s rose up against "C" building staff in hours-long disturbances. In 1955, the Baltimore Afro American linked three major riots over two years to complex causes, and the violent uprisings and press coverage underscored the need for radical transformation in the facility's resources, personnel, and capacity. A major response came through community activism and empowerment within the African American community as church groups and patient families gained access to the hospital and increased public accountability for patient care. Founded in 1954 in collaboration with the NAACP, the Crownville Auxiliary brought white and Black female volunteers to the campus, where they supported and observed day-to-day activities, and the group was instrumental in diversifying Crownsville's mostly white staff and physicians. Under Superintendent Dr. Jacob Morgenstern, a Holocaust survivor who took office in 1947, Crownsville entered a new era of reform, integration, and humanity. In 1948, he hired Vernon W. Sparks as the facility's first Black medical staff member, and he also hired Gwendolyn Lee to lead the Social Services Department, which supported some 1,800 patients in 1949 and became entirely Black during her tenure. More Black staff members were hired, new initiatives reformed standards of patient care, and community and staff empowerment led to much-needed facility growth and improvement, including more modern medical and psychiatric services.
