Crestview Lodge #364 F&AM PHA was chartered in 1904 with Augusta G. Conyers as WM, went inactive in 1942-43 with Will Lewis as WM, was reactivated in October 1944 with K.L. Popleon as WM, and was renamed J.R.L. Conyers Lodge #364 in 1980 with James R.L. Conyers as WM.
